AIN Cognitive Assessment Center
The AIN Cognitive Assessment Center offers a comprehensive evaluation of patients exhibiting signs of cognitive impairment. This is especially important since early diagnosis and treatment can assist in delaying the progression of dementia. Under the direction of board-certified neurologist Nader Warra, DO, we conduct detailed evaluations and address concerns about all forms of dementia, including Alzheimer's disease.
The skilled AIN team will develop personalized treatment options and a patient plan of care designed to improve quality of life. The goal of the AIN Cognitive Assessment Center is to educate the patient and family by connecting them with resources that support the plan of care and provide them with peace of mind.
What to Expect
We understand that many of our patients are new to receiving a cognitive assessment and are unsure what they can expect from their appointments. Our ultimate goal in performing your assessment is to proactively diagnose and treat any conditions that may cause cognitive decline, including Alzheimer's disease and other forms of dementia.

Initial Intake Visit
- Medical history and neurological exam
- Medication review
- Develop a care plan and testing
- Diagnostics, MRI, and EEG on site
- Cognitive testing: CANS, MOCA, referral for neuropsychological testing
- Laboratory: to assess for etiologies or mimickers of memory loss
- Referral for PT evaluation – onsite facilities
- Balance and risk fall assessment
- Onsite MRI, physical therapy, and EEG
Care Planning/Follow-Up/Results
- Comprehensive health report forwarded to primary care physicians
- Coordination of care among all disciplines
- Plan of Care and treatment recommendations
- Treatment options that include traditional medications and new infusion medications
